Please read carefully before purchasing or submitting your urine sample.
This Disclaimer is required for all individuals who choose to purchase laboratory testing offered by Quality Forensic Toxicology, LLC (“QFT”)
By signing below, you acknowledge and agree to all terms stated herein.

1. Purpose of the Test

  • This test is provided for informational and research purposes only.
  • QFT’s role is limited to the laboratory testing and reporting of urine test data.
  • Test results are not intended to diagnose, treat, cure, or prevent any disease and should not be interpreted as medical advice.
  • You agree not to rely on the test or the research interpretation as medical advice and to consult a licensed healthcare professional for any medical concerns.


2. Voluntary Participation

  • You are voluntarily purchasing this test and voluntarily submitting your urine sample.
  • You certify that the sample you submit is your own and that all information you provide is accurate.
  • You understand that improper collection or delayed shipment may impact test quality.


3. Nature of Testing & Limitations

  • QFT performs testing solely according to the UST/Spike Detect methodology provided by B.A.I. Technologies SA.
  • QFT does not determine or modify scientific protocol, targeted analytes, or research interpretation.
  • QFT provides urine test results and raw data only.


4. No Warranties
To the fullest extent permitted by law, QFT makes no warranties, express or implied, including:

  • merchantability
  • fitness for a particular purpose
  • accuracy or completeness of results
  • uninterrupted or error-free testing or reporting

You acknowledge you are not relying on any representation beyond what is expressly written here.

5. Limitation of Liability

  • QFT is not liable for any indirect, incidental, special, consequential, punitive, or exemplary damages, including personal decisions made based on results.
  • QFT is not responsible for any interpretations, conclusions, or recommendations provided by FRI or BAI.


6. Data Use & Privacy

  • You authorize QFT to transmit raw test data to the ZeroSpike/FRI research server.
  • QFT will protect your information in accordance with its policies and applicable laws.


7. Sample Collection & Shipping

  • You are responsible for proper collection, packaging, and shipping.


8. No Medical Emergencies

  • This test must not be used for urgent or clinical diagnostic situations.
  • If you are experiencing concerning symptoms, seek medical care immediately.


9. Agreement to Terms

  • You have read and fully understand this Disclaimer.
  • You voluntarily consent to the testing and data transmission described.
  • You understand QFT’s limited role and the separate research functions performed by FRI.
  • You assume full responsibility for how you use the test results and research interpretation.
  • You release QFT from all liability to the fullest extent permitted by law.
