1) REP ACCOUNT MANAGEMENT PAPERWORK

To establish an account for new clients, utilize the most recent versions of the Account Management paperwork. These PDF’s are fillable, so a majority of the information can be filled out digitally, printed, and then allow for a “wet signature” where required.

2) ACCESSING YOUR EMAIL ACCOUNT

If you are viewing this page, you have been given a qftox.com email account. If you have not accessed your web-based email account, check your personal email for an email from Regina that contains instructions and a temporary password for your account. If you cannot find it, email Darin at dcabell@tzs.b72.myftpupload.com for details.


3) HIPAA TRAINING

Hopefully, you have full access to your new QFT emails and have seen the invitation to Stericycle, where our HIPAA training occurs. Below you will find instructions on how to complete the required Annual HIPAA Training. If you have completed HIPAA training very recently, please reply and it may be acceptable for our records. Please do however make sure you set up your Stericycle account for future trainings.

One note before you continue to the Stericycle instructions. You have probably already received instructions on how to make sure your email communications may be encrypted when sending PHI.

ENCRYPT OR CONFIDENTIAL – ENSURING HIPAA COMPLIANCE

In the subject line, you may use either “encrypt” or “confidential” to make sure the email is encrypted. This ONLY WORKS if it is originating with your QFT domain (qftox.com) email. So, if you are sending patient information or inquiring about an accession # or sample results to another QFT email or an outside domain, your email will be encrypted.

This does not work if you are sending from an outside domain to the QFT domain. For this reason, you will want to use your QFT email accounts (as opposed to any personal or other work email). Please feel free to check in with QFT Support if you have any questions on this.

STERICYCLE INSTRUCTIONS

As part of the normal onboarding process with QFT, we will need you to go through an online training through Stericycle. I will be sending a separate email via the Stericycle website that is an invitation to the site. You will have to fill out a few questions to complete your account set up.

I would suggest utilizing Google Chrome (typically fewer issues), but there are additional directions for other browsers under Technical Support in the menu on the mystericycle.elogiclearning.com page. You will need to be logged in to view it.

SUPPORTED BROWSERS

The Online Training Center is a web-based application that runs through a standard web browser. Internet Explorer 9 and higher, Google Chrome, Safari, and Firefox are the supported browser versions.  Any browser that does not support JavaScript or has it disabled will not function properly. Make sure any web browser you’re using has the web address “mystericycle.elogiclearning.com” listed as an allowed website. In Chrome check the settings for Flash, JavaScript and Popup Blocker to allow this website. If you are not sure how to check your browser settings, please reach out to your company’s technical staff for assistance.

Sections 4 through 6 are required training materials that all QFT employees must view and/or read and track.

4) TOXICOLOGY 101

You will need to be logged-in to your Office365 account to view the Toxicology 101 as a PowerPoint presentation. If you are not, you will prompted for your username and password. UPDATE: A YouTube video has been added below.
